Would you like to register at Dental practice the Wolvenstraat? You are very welcome!


If you fill in the registration form below, we have all the information to register you in our system. After completing the form, we will invite you for an intake. This intake is meant to get acquainted and we will also do a check up on your teeth. Please read the information under the form carefully to prepare for the first appointment. 

 

Due to a lot of urgent registrations we don’t have availability on short term. We have a waiting period of approximately 6 weeks. After that, we would love to welcome you in our practice. For dental cleaning we don’t have a waiting period.

    Health questionnaire

    During your first appointment, you will be asked to fill in the health questionnaire. It is important for us to have this form filled in. Your dentist cannot always take proper precautions if he or she is not sufficiently informed about your answers to the questions on this list. Therefore, we would like to emphasize that it is necessary to answer this questionnaire truthfully.

    Important

    Finally, it is important to note that the practice is located on the first floor and is only accessible by stairs. There is no elevator. The practice is therefore poorly accessible for disabled people.
